Office hours at the Skokie Location are held on Mondays and Tuesdays, and every other Thursday are from 10 a.m. - 5 p.m.

On Wednesdays from 10:00 a.m. - 5:00 p.m., patients are seen at the Buffalo Grove or Gurnee locations, as we alternate sites weekly. Please contact the office directly for appointment information.

Once every other Thursday morning, Dr. Hozman holds office hours at the Lake Forest office from 8:30 a.m. - 12:00 p.m.

Every Friday, patients are seen at the Lake Forest office from 10:00 a.m. - 5:00 p.m.

Dr. Hozman also holds hours on the first Saturday of the month at the Lake Forest location and on the third Saturday of the month at the Skokie location.
